Did you use the suspend obstruction command? If not, the NPCs may be running into each other. That happened to me once.

Sounds like a great idea to me. Maybe you could have it so that if a character tries to cast a spell that is above their ability/spell level, they take damage (from inexperience with that spell).

I like the secret item idea, but right now I'm just going story-wise. I made the guy with extremely high HP and if you do somehow manage to kill him, he just respawns.
